There is no specific form to file provided by the courts however under Federal Arbitration Act, specific documents relevant to the hearing must be included in the petition.
(1) The arbitration agreement
(2)The selection or appointment of the arbitrator
(3) The arbitral award
(4) If there is a written extension for the time within which the award must be made, any such extension
(5) Any notices, affidavits or other papers "used upon an application to confirm, modify, or correct the award," together with each court order on the application
